Homeless Permanent Supportive Housing

Description

Offers three permanent supportive housing programs; Turningpoint Housing Program (THP), Permanent Step Program (PSP) and Project Thrive. All provide rent subsidy in which participants pay 30% of their gross income towards their rent. Clients must meet eligibility requirements and be referred through the coordinated entry system for housing openings.

Hours

Monday-Friday, 8:30am - 5pm

Required Documents

Income verification, certification of disability (within 120 days) and proof of homelessness.

Eligibility

Chronically homeless individuals or families, chronic verifiable disability i.e. mental illness, substance abuse, HIV/AIDS, and physical and developmental disabilities.

Fees

Participant rent portion based on 30% of income, if income is available.

Intake Procedure

Must complete an assessment at a CARE Center in order to be referred into program. No self referrals. Call 2-1-1 for referral to the closest CARE Center.
