Provides outreach to reentry population by assisting with health coverage benefits and SSI/SSA benefits. Health Conductors pre-screen for health coverage insurance while incarcerated, perform follow-ups once individual is released to schedule medical and dental appointments. Offers Transitions clinic which consists of health conductors scheduling medical appointments, appointment accompaniment and advocacy for patient health concerns. Offers REMEDY (Reentry Making Every Day Yours) program which is a cognitive behavioral support group to help clients overcome daily barriers. Connects individuals to other essential services such as housing, substance abuse, medical providers and mental health services.

Description

Provides outreach to reentry population by assisting with health coverage benefits and SSI/SSA benefits.
