Hours
Monday-Friday, 7:45am - 3:30pm; emergency care: call between 7am - 7:30am, some Wednesday evening emergency appointments.
Required Documents
Valid identification and proof of insurance.

Contra Costa resident age 2+
Fees
Medi-Cal/Denti-Cal, CCHP and health insurance for in home care providers.
Intake Procedure
Apply by phone; emergency care: call between 7am-7:30am to be seen same day. Must have referral from CCHS medical provider. No new patients without referral are accepted for dental services.
