Explores available options, services and resources including; home and vehicle adaptation, home set up, assistive devices, wheelchairs and other equipment, training on self-care, nursing visits and how to get a personal care attendant. Patients may be eligible for Medi-Cal California Working Disabled Program, which allows patient to work while still receiving benefits.

Office: 8:30-4pm; Program: Monday-Friday, 9:30-3pm

Eligibility
Medi-Cal eligible individual and has lived in a Medi-Cal paid inpatient facility for at least three months, who wants to move back home or to community living. Client does not have to wait three months to apply.
