Referrals through 211. If CARE center or shelter encounters an individual who is at imminent risk, will refer individual to 211 for referral. 211 provides triage tool.

    Description

    Offers housing navigation services and housing brokerage services to homeless individuals through community referrals and through referrals from the County Coordinated Entry System. Housing Navigators can assist with housing support at local shelters. After residents are housed, Hope Solutions provides support services to help formerly homeless residents maintain stable housing.

    Intake Procedure

    Residents are referred through the CC County Coordinated Entry System. This system can be accessed through the shelter or by dialing 2-1-1 if physically unable to access a shelter or CARE Center.
