Individuals will be informed what documents are needed after the initial screening call.
Eligibility
People living in Contra Costa who: Have a gross household incomes between 139% to 300% of the Federal Poverty Level (FPL). Are not eligible for Medi-Cal or any other health insurance. Are not enrolled in a Health Maintenance Organization (HMO) or Preferred Provider Organization (PPO) health insurance plans. Children under 19 years of age who are eligible for restricted Medi-Cal must apply for the restricted benefit.

Intake Procedure
Call (800) 771-4270 (toll free 7 a.m. - 5 p.m., Monday through Friday) to speak to a Financial Counselor for an initial screening. If it is determined you may be eligible, you will be mailed an application and a letter requesting additional documentation.
