Description:

Collaborates with an array of service providers including community-based and faith-based organizations, county agencies, and local law enforcement to ensure successful reentry and reintegration for residents returning home from incarceration to Central and East Contra Costa County. Reentry service include an assessment of individual's needs and reentry planning, mentor and peer support, system navigation and coordination of resources including, but not limited to housing assistance, education and employment preparation, vocational training and job placement, legal assistance, family and community reunification, etc. Field Operation Coordinators (FOCs) are mobile staff that conduct face-to-face contact with individuals at multiple convenient locations known as No Wrong Door sites. FOCs are the points of contact for community partners and residents in need of access support through the Reentry Network. The Network also works with volunteer Mentors to support returning residents and their families.

Eligibility

All probationers with an AB109 status either residing in or returning to Central and East Contra Costa County. All other formerly incarcerated individuals in Contra Costa County, under supervision or not, will be assessed on an individual basis.
