Monday-Friday, 6am-7pm PST
Required Documents
Proof of income and social security card.
Eligibility
Family income is no more than 200% of federal poverty level; child is not enrolled in Medi-Cal or other vision insurance, child is 19 years old or younger, valid social security number, child has not used the program during the past 12 months.
