Monday-Friday, 8am - 4:30pm
Required Documents
Valid identification, proof of health insurance and medical records.
Eligibility
Individuals experiencing significant memory loss
Fees
Accepts Medicare, MediCal and most PPO; all HMO insurances require a referral and authorization prior to being seen. Private pay accepted.
Intake Procedure
Apply by phone, medical records will be requested before an appointment is made; no walk-ins.
