Provides assistance to identify homebound patients who require COVID-19 vaccine administration at home. Completes interest form on behalf of homebound patients in order for Contra Costa County Public Health to administer a vaccine at patients home.

Description
Offers free health insurance counseling for anyone on Medicare and seniors age 60+ at sites throughout the county. Assistance is provided by trained volunteers for help with Medicare, Medi-Cal billing procedures and evaluating the various supplemental plans including HMOs, Medigap and long-term care insurance. Counseling is done at various senior centers, medical centers and community facilities in the county. No insurance is sold or endorsed. Community education on Medicare and long term care insurance through presentations and forums.
