Description:

Provides Screening, Diagnostic and Treatment (EPSDT) counseling for children, adolescents, families and groups age 5 to 21. EPSDT process is in place to assure that medically necessary health care is received in order to correct or ameliorate a defect, physical or mental illness, or a health condition identified through a screening. Services offered at clinic site as well as schools within the WCCUSD. Offers substance abuse treatment for adolescents age 14-18. Works with court referred youth on probation for mental health and/or Alcohol and other drugs (AOD) related treatment.

Description

Provides community-based outpatient substance abuse treatment and co-occurring behavioral health services for adolescents. Services are available for court-referred and other youth who are on general probation supervision or have been court-ordered for treatment. Program offers targeted case management and family counseling.

Description

Provides high fidelity wrap around model assistance that helps the family and child coordinate services and supports in a way that empowers them to meet their needs as they define them. Partners with families to develop a family-driven plan that promotes and helps with self-advocacy, system navigation, referrals for housing and employment as well as substance abuse issues.
