Provides screening and enrollment for CARES, which is a free primary care coverage to adults living in Contra Costa County that do not qualify for other health insurance. Enrolled participants have access to preventative care services, free of charge, at either Brighter Beginnings, La Clinica de La Raza, or LifeLong Medical Care. There are no monthly premiums or copays.
