Description

Required Documents

Doctor must complete the medical certification of individual's disability. The certificate must include a diagnosis, beginning date of the disability, probable duration and the estimate time care is needed. An estimate of the amount of time necessary to provide care is also required.

Eligibility

California resident, covered under SDI, who must take time off to care for a seriously ill child, spouse, parent, or registered domestic partner, or to bond with a new child or with a child in connection to an adoption or foster care placement. Individual must have earned at least $300 from which SDI deductions were withheld during a previous period and a doctor must complete the medical certification for the reason for the leave.
