Description

Provides permanent affordable housing solutions and vital support services to homeless and very low-income families and individuals in Contra Costa County. Partners with Mercy Housing to provide Permanent Supportive Housing for the most vulnerable at Garden Park Apartments (27 family units, Pleasant Hill). Also provides Permanent Supportive Housing in a multi-site program throughout the county (Richmond to Antioch) to 20 additional families and 48 individual adults. All Permanent Supportive Housing units are filled through the CC Coordinated Entry system. Homeless individuals can be placed on the Coordinated Entry list at the CARE Centers.

Description

Partners with the Contra Costa County Employment & Human Services Department (EHSD) and the County Health, Housing and Homeless Services Department (H3) to provide the HousingWORKS! Housing Support Program (HSP): HousingWORKS! provides supportive housing services to homeless CalWORKs families to help them achieve self-sufficiency and housing stability. Supportive services include assistance with housing search and temporary/permanent housing placement, time-limited subsidy assistance using Rapid Rehousing and Housing First models and on-demand in-home services. Eligible families are referred through EHSD and the H3 County Coordinated Entry System. Offers Youth and Young Adult (YYA) Housing Support program which provides time-limited subsidy assistance and in-home support services to up to 40 transition age youth to help them achieve permanent housing and independence the CC County Coordinated Entry System.
