Individual 55+ with medical conditions that make it difficult to live at home without supportive services.
Fees
Accepts Medi-Cal, Medicare, and private pay.
Intake Procedure
Call for services, home visit appointment through telehealth will be scheduled for an assessment. Tour of local center and enrollment for eligible clients by appointment.
Languages
English, Spanish, Chinese, Korean, Vietnamese, and Tagalog; interpreter services available for all other languages.
