Monday-Friday, 8am - 5pm
Required Documents
Valid of identification, 12 month income verification, Childs original birth certificate, immunizations including TB test for child 1 year or older including the immunization result. Proof of health insurance, and two verifications of address.
Eligibility
Income eligible family with children age 0-5.
Fees
None for income eligible client.
Intake Procedure
Call to schedule an appointment to complete an application.
