Program offers confidential, free, and contactless rapid HIV and gonorrhea and chlamydia testing in your home/private space. Description
Provides counselors to help HIV-positive individuals explore ways to tell their partner (sex and/or needle sharing) about their possible exposure so that partners can make informed decisions about their health. The counselor provides sensitive and private support, answers any questions and concerns, explores options with clients about disclosure. Clients determine the various options they want to use to inform their partners. Call for more information or private appointment.

Description
Provides a variety of services for HIV-infected individuals including medical case management, women's services, home health care; referrals to physicians experienced with HIV infection; help with transportation, food, housing/utility and other emergency assistance; and information and referral for other needed resources (such as substance abuse services, mental health therapy, adult day center care, etc.). Provides ADAP medications for financially eligible persons and PrEP navigation services (prevention) for HIV negative individuals.
