California resident who is temporarily or permanently: deaf/deafened, hard of hearing, cognitive, mobility, blind/low vision or speech, are eligible to receive equipment.

Intake Procedure
Call for information or download application online. Must have application authorized by certifying agent; medical doctor, a licensed audiologist, a licensed optometrist, a qualified state agency, licensed physician assistant, licensed speech-language pathologist or a hearing aid dispenser.
